Performs the primary functions of a LPN under the supervision of an RN by effectively using basic nursing knowledge, judgment and skills to care for patients in the department during his/her shift is responsible for meeting the established Standards of Nursing Practice and The Practice Act, by providing appropriate care to all assigned patients.
Duties and Responsibilities:
Demonstrates the ability to accurately gather and record health information for patients/residents, correctly identify physical and psychological needs, and implement appropriate nursing actions collaboratively under the direction of an RN.
Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital policies and nursing standards of care.
Promotes patient's independence by establishing patient care goals; teaching patients and families to understand conditions, medications, self-care skills, and answering questions. Monitoring patient's progress carefully, intervenes as able or notifies the RN if intervention is needed outside the LPN scope of practice.
Demonstrates competency in implementing and maintaining IV therapies according to policy and scope of practice.
Maintains privacy and security of all Protected Health Information (PHI) and is sensitive to personal privacy and security issues.
Provides exceptional customer service and demonstrates the ability to resolve patient problems and needs by utilizing multidisciplinary team strategies.
Maintains safe and clean working environment by complying with procedures, rules, regulations, calling for assistance from health care support personnel.
Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
Promotes hospital improvement activities and own professional development. Includes certifications, licensure, monthly staff meetings, and annual in-service, CEUs and hospital events.
Establishes a compassionate environment by providing appropriate emotional, psychological, and spiritual support to patients and families.
Required Minimum Qualifications :
Education:
Successful completion of Licensed Practical/Vocational Nursing program from an accredited college or university.
