Job Description
Principal Function:
The Maternal Health Nurse/Care Coordinator is responsible for coordinating care from pregnancy through postpartum and infant care.
Essential Duties and Responsibilities:
Duties include, but are not limited to: Assist with the implementation and refinement of the Maternal Women's Health program (NEST). Coordinates care from pregnancy through postpartum and infant care. Track and monitor abnormal results, missed care, and referrals through the full follow-up cycle. Identify system changes that close gaps and improve outcomes for historically underserved maternal and neonatal populations. Track every patient handoff until the patient receives the service and the care teams know the results. Define and track imaging/ultrasound workflow, who responds to urgent findings, how staff obtain specialist advice, when and where a patient is transferred, how communication is documented, and how quality is reviewed. Translate registry findings into actionable strategies for improving protocols, resource allocation, and maternal/infant health policy. Develop and disseminate tailored reports and dashboards. Attend community meetings to build relationships with hospitals, health departments, WIC, and other maternal infant partners. Assist with the development of patient education material no fetal development, labor signs, newborn care, and postpartum mental health.
Measure access and completed care:
track how early prenatal care begin, whether recommended prenatal visits are completed; postpartum planning and visits, completion of ultrasounds and laboratory testing, and completed referrals. Improve access and follow-up. Reduce referrals that do not reach completion, shorten the time from pregnancy identification to the first visit, and test the handoff from pregnancy care to postpartum and infant care. Complete a maternal safety review by checking abnormal results, urgent situations, transfers, missed visits, medication reviews, interpreter use, and follow-up to make sure the approved process was followed. Experience:
Bachelor of Science in Nursing (BSN) or Associate of Science in Nursing (ASN) or Associate Degree of Nursing (ADN) or equivalent from a two-year college or technical school. Previous experience in obstetrics or women's health is desired. Additional required licenses and certification include: Current Indiana license for Registered Nurse (RN) or Licensed Practical Nurse (LPN) Certification or the ability to obtain and maintain a certification in Cardiopulmonary Resuscitation.
