Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and safety through comprehensive assessments, medication administration, and personalized care plans. Hour Mid Shifts Pay:
upto $45/hr Max Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Assist with diagnostic procedures such as specimen collection, catheterization, and basic medical imaging support. Manage IV infusions and ventilator support in accordance with clinical guidelines and safety standards. Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or memory impairments. Document all patient interactions meticulously within Electronic Health Record (EHR) systems while maintaining compliance with HIPAA regulations. Support discharge planning by coordinating with multidisciplinary teams to ensure seamless transitions of care. Educate patients and their families on health management strategies, medication use, and post-treatment care. Implement infection control procedures and aseptic techniques to uphold safety standards across all clinical activities. Collaborate effectively with physicians, therapists, case managers, and other healthcare professionals to optimize patient outcomes. Experience Proven experience working as an LPN in diverse healthcare environments such as hospitals (including ICU, emergency room, trauma centers), nursing homes, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth. Critical care experience including ICU or Level I Trauma Center exposure is highly desirable. Experience managing patients with complex needs such as acute pain management, infectious diseases, or post-operative recovery. Familiarity with medical coding systems including ICD-9/10 and CPT coding for documentation accuracy. Knowledge of long-term care regulations and geriatric patient management principles. Skilled in performing medical procedures such as venipuncture, suturing assistance, catheter care, and tube feeding. Prior experience working with individuals with developmental disabilities or behavioral health conditions is advantageous. Strong understanding of physiology, anatomy knowledge, and clinical confidentiality policies necessary for safe practice. Ability to adapt quickly in fast-paced settings such as urgent care or medical-surgical units while maintaining high standards of patient care. Prefer someone with Matrix experience but will train
LTC LPN/RN
candidates must have 2 years of recent LTC experience. Must have BCLS and an Active
IN LPN/RN
