Job Description

Encompass Health Cardinal Hill Rehabilitation Hospital is seeking a dedicated LPN/LVN to join our Healthcare & Medical Services team. In this role, you will provide high-quality, compassionate care to rehabilitation patients, administer medications, monitor patient progress, and collaborate closely with an interdisciplinary team. Responsibilities Provide direct bedside nursing care to adult and geriatric rehabilitation patients under RN and physician supervision Administer medications and treatments safely and accurately according to physician orders and facility policies Monitor, record, and report patient vital signs, responses to treatment, and changes in condition in a timely manner Assist with patient admissions, transfers, and discharges, including initial assessments and patient orientation Document all care, interventions, and patient education accurately in the electronic medical record Support patient mobility, positioning, and activities of daily living while reinforcing therapy and rehabilitation goals Collaborate with RNs, physicians, therapists, and case managers to implement and update individualized care plans Educate patients and families on medications, treatment plans, safety precautions, and discharge instructions Maintain infection control, patient safety, and regulatory compliance at all times Participate in team huddles, shift reports, and quality improvement initiatives to enhance patient outcomes Required Skills Medication administration and dosage calculation Wound care and basic dressing changes Vital sign monitoring and patient assessment at the LPN/LVN scope Rehabilitation and mobility support techniques (transfers, ambulation, use of assistive devices) Electronic medical record (EMR) documentation Infection control and aseptic techniques Basic IV therapy skills (per state scope and facility policy) Patient and family education and communication Time management and patient prioritization in a fast-paced setting Interdisciplinary team collaboration Basic cardiac and respiratory monitoring recognition within LPN/LVN scope of practice
