In this vital role, you will provide exceptional patient care across diverse clinical settings, ensuring safety, comfort, and optimal health outcomes. Your expertise will support a wide range of medical services, from acute care to long-term management, empowering patients on their journey to wellness. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews Administer medications and injections accurately following prescribed protocols, including IV infusions and injection administration Utilize EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for documentation and medical records management Perform specialized procedures such as suturing, catheterization, tube feeding, ostomy care, and sterile processing when required Coordinate patient care plans with multidisciplinary teams, including case management and behavioral health specialists Support triage processes by evaluating patient symptoms and determining urgency levels for emergency room or urgent care settings Educate patients and families on health coaching, medication management, discharge planning, and chronic disease management Qualifications Valid nursing license with current registration in the relevant jurisdiction Proven experience in diverse clinical environments such as hospital medicine, ICU/CCU (Critical Care Unit), emergency medicine, or long-term care facilities Strong proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with
ICD-9/ICD-10
coding is preferred Knowledge of advanced nursing procedures including ventilator management, laboratory specimen collection/processing, and aseptic technique Experience working with individuals with developmental disabilities or in memory care settings is advantageous Ability to perform physical examinations and diagnostic evaluations with attention to detail Excellent communication skills combined with critical thinking and problem-solving abilities Join us to be part of a passionate team committed to delivering outstanding patient-centered care.
