The Medical Team , a full-service healthcare organization, is seeking experienced and compassionate LPN (Licensed Practical Nurse) to work with our Personal Care Service division. The Medical Team provides personal care for seniors in independent and assisted living facilities (such as; showering assistance, toileting assistance, med passing, escorting to and from the dining area, and help dressing and grooming).

Essential Functions:

Regularly monitor medication cart and perform weekly med cart audits. Participate in arranging and conducting orientation / in-services for nursing assistants/ patient care attendants. Oversee client's services are being provided in accordance with task assignments and conforming to Agency policy. Ensure clients needs are being met thru various non-clinical county programs. Participate in Blood Pressure Clinics, Flu Immunization Clinics, Lab Draw Clinics, and Health Fairs as directed. Redirect evidence of skilled care needs to clinical Division as indicated for appropriate follow up. Assist with Medicare overflow visits when able. Supervisory visits for all CNA/PCA at building quarterly. Fulfill additional performance responsibilities as assigned and as necessary.
