Job Summary Harmoni Care Solutions is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide In-Home Nursing Respite (IHNR) services to an adult client in Olive Branch, MS. The client has a profound intellectual disability, cerebral palsy, and a seizure disorder with daily seizure activity. He uses a wheelchair and gait belt and requires total assistance with activities of daily living and constant supervision. This is a full-time position, Monday-Friday, 8:30am-4:30pm, providing skilled nursing support and personal care in the client's home. Responsibilities Provide constant supervision to keep the client safe from harm, including keeping him away from the stove, hot foods, and items he could pull down on himself Monitor for seizure activity, including petit mal and grand mal seizures, and time and document every seizure Implement seizure precautions per care plan, including safe positioning during and after a seizure and monitoring his breathing Administer rescue medication (Diastat) for seizures lasting more than five minutes per physician order, and follow emergency procedures Administer scheduled and PRN medications on time per physician orders, including crushing medications as ordered and giving them with small amounts of fluid Monitor for aspiration during meals and fluids; serve small food portions and limited drink amounts per care plan Prepare meals and provide total assistance with feeding Provide total assistance with bathing, oral hygiene, grooming, and dressing Provide total assistance with toileting every two hours, including incontinence care Perform safe transfers using a gait belt and wheelchair, and provide close supervision because he may try to stand on his own Reposition the client every two hours to relieve pressure, and monitor skin for breakdown or bed sores Assist with range of motion (ROM) exercises to relieve heel cord stiffness Administer nebulizer treatments as ordered, and monitor for respiratory problems and allergy symptoms Monitor bowel habits and give PRN medication for constipation as ordered Follow the client's behavior support plan, and use redirection for physical aggression (grabbing, slapping, pulling, pushing, pinching) and self-injurious behavior Follow the client's daily routine and explain each task before doing it, as predictability helps reduce his anxiety Support meaningful activities he enjoys, such as music, singing, videos, picture books, and wheelchair walks outside when the weather permits Keep the client's areas of the home clean and orderly Monitor and document the client's health status, vital signs, and any changes in condition Report changes in the client's condition to the supervising RN and/or care team promptly Maintain accurate and timely documentation of services provided Follow the individual's approved care plan and Harmoni Care Solutions' policies and procedures Communicate professionally with the client, family members, and care team Qualifications Current, unencumbered Mississippi LPN license (or multistate compact license) Current CPR certification Experience with seizure disorders, including rescue medication administration, preferred Experience with cerebral palsy, aspiration precautions, and total-care clients preferred Experience supporting individuals with IDD and behavioral needs preferred Ability to perform safe transfers with a gait belt and wheelchair Patient, calm, and consistent approach Clear background check Reliable transportation Schedule Monday-Friday, 8:30am-4:30pm (40 hours per week)
