Job Description

$10,000 Sign on Bonus Job Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where you will play a vital role in delivering compassionate, high-quality patient care across diverse clinical settings. This energetic position offers the opportunity to work closely with patients ensuring their comfort, safety, and well-being. As an LPN, you will utilize your clinical expertise to assess patient conditions, administer medications and treatments, and support interdisciplinary teams in providing care. Your dedication will directly impact patient outcomes and enhance the quality of life for those we serve. Duties Perform comprehensive patient assessments by taking vital signs, conducting physical examinations, and evaluating physiological responses to treatment plans. Administer medications, including injections and IV infusions, following strict protocols to ensure safety and efficacy. Manage complex care procedures such as catheterization, tube feeding, wound suturing, and ventilator management for patients with acute or chronic conditions. Document all patient interactions accurately using Electronic Health Records (EHR) system like MatrixCare while maintaining strict compliance with HIPAA and clinical confidentiality policies. Assist with diagnostic evaluations such as specimen collection, processing laboratory tests, and supporting medical imaging procedures. Provide specialized care for patients with dementia or Alzheimer's disease in memory including behavioral health support and dementia care strategies. Collaborate with physicians and interdisciplinary teams in discharge planning, case management, and long-term care regulation adherence to optimize patient outcomes across settings like skilled nursing facilities Experience Proven experience working as a Licensed Practical Nurse in diverse clinical environments such as hospitals, nursing homes, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks for accurate medical documentation and patient record management. Critical care experience including ICU or emergency room settings is highly desirable; familiarity with Level II trauma center protocols is a plus. Hands-on experience administering IV infusions, managing acute pain cases, performing triage assessments, and providing urgent or emergency care. Knowledge of ICD coding (ICD-9/10), CPT coding for procedures, infection control practices, and long-term care regulations enhances your ability to deliver compliant patient services. Experience working with individuals with disabilities or developmental delays; familiarity with hospice care and palliative approaches is advantageous. Strong understanding of physiology, anatomy knowledge, medication administration techniques, and physical examination skills necessary to deliver safe patient-centered care across multiple specialties including geriatrics, pediatrics, internal medicine or behavioral health.
