Monitor resident condition and promptly communicate changes to the healthcare team. Accurately document resident care and interventions. Collaborate with physicians, nurses, therapists, and interdisciplinary care teams. Follow infection prevention, safety, and regulatory standards. Graduate of an accredited Practical Nursing program. New graduates are encouraged to apply. Experience in skilled nursing, long-term care, rehabilitation, or post-acute care is a plus but not required. Strong communication, critical thinking, and teamwork skills.
