Clinical Manager/Director of Nursing Position Summary We are seeking a Licensed Practical Nurse (LPN) to join our home health and hospice agency in an office-based clinical support and coordination role . This position is ideal for an organized, detail-oriented LPN who enjoys working collaboratively with field clinicians, patients, providers, and agency staff. The LPN will play a key role in ensuring that patient care is coordinated efficiently and that field clinicians have the administrative and clinical support they need to provide high-quality care in the home. Key Responsibilities Patient & Staff Scheduling Coordinate and maintain daily patient and clinician schedules for home health and hospice services. Assign visits based on patient needs, clinician availability, geographic location, and clinical priorities. Adjust schedules as needed for call-outs, changes in patient condition, admissions, discharges, and other scheduling needs. Communicate schedule changes promptly with patients, caregivers, and field staff. Help ensure timely completion of required visits and services. Clinical Order Entry Enter and process physician/provider orders accurately and in a timely manner. Enter and coordinate medication, wound care orders, treatment changes, and other clinical orders. Monitor orders for completeness and follow up on missing or unclear information. Assist with ensuring orders are communicated appropriately to the field clinicians and incorporated into the patient's plan of care. Field Clinician Support Serve as an office resource for field nurses, aides, therapists, and other clinicians. Assist clinicians with contacting physicians, nurse practitioners, physician assistants, pharmacies, and other healthcare providers. Facilitate communication between field staff and provider offices regarding patient status, orders, medications, symptoms, and changes in condition. Follow up on pending orders, medication issues, and other clinical needs. Help troubleshoot patient-care issues and escalate concerns to the appropriate clinical leader when necessary. Communication & Coordination Communicate professionally with patients, families, caregivers, providers, pharmacies, and agency staff. Maintain timely and accurate documentation of communications, orders, and scheduling activities. Assist with coordinating admissions, recertifications, discharges, and other patient-care transitions as needed. Support the clinical team in maintaining continuity of care across the home health and hospice settings. Qualifications Current, unrestricted NH LPN license . Previous experience in home health and/or hospice setting preferred. Strong understanding of medications, wound care, physician orders, and clinical terminology. Excellent organizational and time-management skills. Strong communication and customer-service skills. Ability to prioritize multiple tasks in a fast-paced environment. Comfortable communicating with physicians, provider offices, pharmacies, patients, families, and field clinicians. Proficiency with electronic medical records (EMR) and Microsoft Office or similar software. Ability to work independently while functioning effectively as part of a multidisciplinary team. Ideal Candidate The ideal candidate is a clinically knowledgeable and highly organized LPN who can serve as a dependable resource for the field team. You should be comfortable juggling competing priorities, communicating with multiple parties throughout the day, following up on outstanding orders, and recognizing when a clinical concern needs to be escalated. This is an excellent opportunity for an LPN who enjoys the clinical side of nursing but is looking for an office-based position with a strong focus on coordination, communication, and supporting patient care .
