Shift 1 Job Description Delivery of Nursing Care. The LPN will gather information based on the Nursing Process. The LPN is to deliver a standard of care based on hospital practices, policy and procedure including but not limited to: gathering data such as vital signs, patient signs and symptoms and report to the RN; assisting in carrying out the patient's plan of care; providing on- going Nursing Care and Teaching through the provision of Care supportive to restore life and well being; notifying the RN of any adverse changes in the patient's condition; providing patient education; timely and accurately document care in accordance with licensure, regulatory and hospital standards; identifying patient conditions to achieve positive outcomes; accepting delegation from the RN; utilizing age specific and universal precautions at all times; properly administering medications; providing nursing care treatments; following all hospital policy and procedures;Safety and Environment of Care. The LPN will maintain a safe and clean working environment by complying with procedures, rules, and regulations by, including, but not limited to: calling for assistance from health care personnel when necessary; ensuring operation of equipment by completing preventative maintenance requirements, following manufacturer's instructions, troubleshooting malfunctions, calling in repairs, maintaining equipment and techniques; protecting patients and employees by adhering to infection control policies and protocols, medication administration and controlled substance regulations; utilizing universal precautions and practice at all times;Patient Care Administration. The LPN will document patient care service and communicate appropriately by, including, but not limited to: charting in patients' medical records accurately and timely in accordance with licensure, regulatory hospital standards; maintaining continuity of care with the nursing team by communicating plan of care and follow through with the RN and other multidisciplinary teams ensuring that patient administration of care for all services provided are delivered; maintaining patient confidence and protect operations by keeping information confidential;Continuing Education. The LPN will maintain professional and technical knowledge by, including, but not limited to: attending educational workshops and mandatory in-services, reviewing professional publications, establishing personal networks, participating in professional societies;Nursing Quality Improvement. The LPN will assist in the quality improvement initiatives by including, but not limited to: continuing education on the delivery process and protocol; participating in hospital-wide and unit specific teams; identifying nursing practice areas for quality improvement and safe outcomes; maintaining a cooperative relationship among the healthcare team by communicating information, responding to requests, building rapport; participating in team problem solving;Service Excellence. The LPN provides service excellence to patients and their family members/visitors that meets or exceeds expectations by, including, but not limited to: offering empathetic and compassionate patient-centered care; professionally managing patient and family/visitor needs and concerns using AIDET and positive services recovery methods; responding to patient requests and making active referrals to subject matter expert/decision maker who can fulfill the patient request; providing culturally sensitive care.

Education And Credentials Current NY licensure as a LPN of an accredited school of Nursing.

Experience One or more years Medical Surgical Hospital nursing.
