Job Overview We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to join our healthcare team on a PRN (as-needed) basis. In this vital role, you will provide compassionate, high-quality nursing care to diverse patient populations across various settings. Your expertise will support patient recovery, comfort, and safety while collaborating closely with interdisciplinary teams. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems. Administer medications and injections accurately following physician orders, including catheter and wound care. Manage acute pain and provide specialized care for patients with dementia, Alzheimer's disease, or memory impairments in memory or long-term care sett Document all patient interactions thoroughly in compliance with HIPAA regulations and medical documentation standards. Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans, discharge planning, and case management strategies. Skills Proven experience working with diverse populations including geriatric patients, or those requiring hospice or palliative care. Strong knowledge of medical terminology, anatomy, physiology, and pharmacology related to acute and chronic conditions. Proficiency in EMR/EHR systems such as Cerner, Epic, Athenahealth, or eClinicalWorks for accurate documentation and communication. Skilled in sterile technique to ensure patient safety. Excellent caregiver communication skills with the ability to educate patients and families effectively about treatment plans and health coaching. Ability to perform physical examinations, vital signs monitoring, intake assessments, and triage efficiently. Knowledge of long-term care regulations including skilled nursing facility standards and working with geriatric or pediatric populations.
