Role:
Join our team of dedicated professionals at Better at Home, committed to providing exceptional care to our patients in the comfort of their homes. We are seeking compassionate full-time and part-time Licensed Practical Nurses (LPNs) to join our Henryetta care team. Working collaboratively with our supervising Registered Nurses, your expertise will make a significant impact on the lives of those we serve by helping local patients safely manage chronic conditions and achieve their optimal health at home.
Position Details:
Full-Time:
Approximately 25-30 visits per week
Part-Time:
Variable schedule based on patient needs and availability
Compensation:
$43.00-$45.00 per visit
Key Responsibilities:
Provide skilled nursing care in the home as ordered on the plan of treatment and nursing care plan. Evaluate the significance of assessment findings and communicate pertinent information about the patient's clinical status and ongoing needs to the care team in a timely manner. Adequately assess and reassess pain, utilize appropriate pain management techniques, and educate patients and families regarding pain management. Maintain accurate medical documentation and update medical records in a timely manner. Collaborate with healthcare teams to develop and implement individualized care plans. Educate patients and their families about health management and disease prevention.
Requirements:
Current LPN license in the state of Oklahoma in good standing. Graduate of an accredited practical nursing program with current CPR/BLS certification. Knowledge of pain management techniques and patient care best practices. Familiarity with medical documentation practices and electronic health record (EMR) systems is preferred. Strong interpersonal skills with the ability to build rapport with patients and families. Valid driver's license and reliable transportation for local home visits.
