Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong commitment to providing high-quality patient care in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments. As an LPN, you will play a vital role in supporting patients' health and well-being while collaborating with healthcare professionals to ensure optimal care delivery. Duties Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities. Conduct patient assessments and monitor vital signs to evaluate health status. Document patient information accurately in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations. Collaborate with registered nurses and physicians to develop and implement individualized care plans. Assist in triaging patients and managing acute pain as needed. Maintain a clean and safe environment for patients by adhering to infection control protocols. Participate in discharge planning and case management for continuity of care. Educate patients and families on health management strategies and resources available. Experience Candidates should possess the following skills and experiences: Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation practices. Understanding of anatomy and physiology relevant to patient assessment and care. Skills in medication administration, specimen collection/processing, and diagnostic evaluation. Previous experience with pain management techniques and infection control practices is advantageous.
