Community Care Coordinator/LPN Wewoka, OK From $23 an hour - Full-time From $23 an hour - Full-time East Central Oklahoma Family Health Center, Inc. (ECO Health) has an immediate opening for a Community Care Coordinator/LPN to work at the ECO Health - Wewoka Clinic . Applicant must have strong computer skills and clinical experience is preferred. This is a full time position Monday-Friday.
General Summary of Duties:
The Community Care Coordinator/Licensed Practical Nurse (CCC-LPN) is an integral member of the interdisciplinary health care team and assists in the care of patients, including administration of treatments and medications, patient education, triage and clinic flow processes.
Essential Functions:
Coordinates continuity of care within the patient care team by frequent communication to ensure management of screening gaps, follow-up care and patient education. Assist the provider/care manager in care plan development and updates as needed. Performs Health Risk Assessment on patients. Participates in accreditation and quality improvement projects. Timely follow-up on patients who have had a recent ER visit/acute care hospitalization. Be an active member of the patient care team. Assisting referral coordinator with closing the loop on open referrals. Monitors and completes all task assigned by the healthcare team. Triage of nursing calls to provide patients with timely and efficient health care needs. Monitors scanning file and attaches documents to appropriate order/folder. Assists in scheduling annual wellness visits as well as annual well-child visits. Works with local health departments to conduct assessment of vaccine needs in the community. Must be able to function in the role of LPN clinic nurse. Knowledge of the scope of practice for all nursing disciplines within the Nurse Practice Act. Demonstrates the ability to effectively organize and prioritize work and utilize available. resources to assist with efficient patient flow and promote positive patient outcomes. Appropriate written and communication skills. Good public communications. Performs other duties as assigned.
Qualifications:
LPN currently licensed to practice in the state of Oklahoma required. Current BLS or ability to obtain within the first 3 months of hire. Preferred experience in ambulatory care.
