Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with physician orders, facility policies, state and federal regulations, and accepted standards of nursing practice. The LPN is responsible for supervising nursing assistants, administering medications and treatments, monitoring resident conditions, and contributing to the overall quality of care and resident satisfaction. Essential Duties and Responsibilities Administer medications, treatments, and therapeutic interventions as prescribed. Perform resident assessments and monitor changes in condition. Notify physicians, responsible parties, and nursing leadership of significant changes in resident status. Document nursing care accurately and timely in the medical record. Participate in the development and implementation of resident care plans. Supervise and direct Certified Nursing Assistants (CNAs) and other assigned staff. Ensure compliance with infection prevention and control practices. Respond appropriately to emergencies and initiate necessary interventions. Receive and give shift reports to ensure continuity of care. Maintain resident confidentiality and protect resident rights. Collaborate with physicians, therapists, dietary staff, social services, and other interdisciplinary team members. Assist with admissions, transfers, and discharges. Participate in quality assurance and performance improvement activities. Maintain a safe, clean, and organized work environment. Perform other duties as assigned. Qualifications Education Graduate of an accredited school of practical nursing. Licensure Current Licensed Practical Nurse (LPN) license in the State of Oregon. Current CPR/BLS certification required. Experience Previous experience in long-term care or skilled nursing preferred. Experience with electronic medical records preferred. Knowledge, Skills, and Abilities Knowledge of nursing principles and practices. Ability to supervise and direct nursing assistants effectively. Strong communication and interpersonal skills. Ability to prioritize and manage multiple tasks. Knowledge of state and federal regulations governing skilled nursing facilities. Ability to maintain confidentiality and exercise sound clinical judgment. Physical Requirements Ability to stand, walk, bend, and lift up to 50 pounds with assistance. Ability to push, pull, and transfer residents utilizing proper body mechanics and equipment. Ability to work extended shifts and respond to emergencies
