- teams Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves performing patient assessments, administering medications and treatments, and supporting the overall health and well-being of diverse patient populations. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories. Administer medications, injections—including IV infusions—and treatments following physician orders and safety protocols. Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns appropriately. Maintain accurate medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA regulations. Support discharge planning by providing patient education on medication management, care procedures, and follow-up requirements. Manage catheter care, wound dressing changes, and other specialized nursing interventions. Collaborate effectively with interdisciplinary teams including physicians, therapists, caregivers, and families to coordinate comprehensive care plans. Ensure infection control practices are followed rigorously to maintain a sterile environment. Participate in ongoing training related to emergency medicine, trauma response, dementia care—including Alzheimer's and memory care—and other specialized areas. Skills Valid licensure as a Licensed Practical Nurse (LPN) with current certification in CPR/BLS. Experience working with diverse patient populations across settings such as long-term care facilities, hospitals—including ICU and trauma centers—and outpatient clinics. Proficiency in EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Strong knowledge of medical terminology, physiology, anatomy, and pharmacology relevant to nursing practice. Skilled in medication administration techniques including injections and IV insertions; experience with IV infusion therapy is highly desirable. Familiarity with ICD coding (ICD-9/ICD-10), CPT coding for procedures, and documentation standards for compliance. Ability to perform physical examinations and vital sign assessments accurately. Knowledge of infection control protocols and aseptic techniques to prevent healthcare-associated infections. Excellent caregiver communication skills with the ability to educate patients about their health conditions effectively. Capable of working under pressure in fast-paced environments such as emergency rooms or urgent care centers while maintaining attention to detail. Experience working with individuals with disabilities or complex health needs such as dementia or behavioral health conditions is preferred.
