Job Description

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. As a Part Time Licensed Practical Nurse (LPN) in Chartiers Center's residential program, you will play a key role in supporting the health, wellness, and recovery of up to 16 residents living with mental health challenges. In this rewarding position, you will provide compassionate nursing care, monitor residents' physical and behavioral health needs, administer medications, coordinate with treatment teams, and help promote a safe, supportive, and therapeutic living environment. Working closely with psychiatrists, therapists, residential staff, and other healthcare professionals, you will make a meaningful difference in the lives of individuals as they work toward greater independence, stability, and recovery. This is an excellent opportunity for a caring and dedicated nurse who is passionate about serving others, building positive relationships, and contributing to a team that is committed to person-centered care and helping residents achieve their goals. Job responsibilities include the following : Administering medications accurately and safely while monitoring residents for effectiveness, side effects, and overall well-being. Maintaining accurate, timely, and confidential health records and documentation to ensure current and comprehensive resident health profiles. Completing nursing and medical components of treatment plans, including assessments, health monitoring, and coordination of care. Collaborating with psychiatrists, physicians, therapists, residential staff, and other interdisciplinary team members to support residents' medical and behavioral health needs. Responding to health concerns, providing education and support to residents, and promoting healthy lifestyle choices. Contributing to a safe, caring, and recovery-focused environment that empowers individuals to achieve their personal recovery goals and improve their overall quality of life. Daylight, evenings, and/or weekend hours.
Minimum Qualifications:
