Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality resident care to long-term care residents. Your expertise will help ensure residents receive personalized treatment, comfort, and support while maintaining compliance with healthcare standards and regulations. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse resident populations, including seniors, individuals with developmental disabilities, and those requiring specialized care such as Alzheimer's or memory care. Duties Perform comprehensive resident assessments by monitoring vital signs, conducting physical examinations, and evaluating resident needs to inform care plans. Administer medications and injections accurately following prescribed protocols. Assist with complex procedures such as catheterization, tube feeding, and sterile dressing changes using aseptic techniques. Manage resident documentation efficiently within Electronic Health Record (PCC) to ensure accurate medical records and compliance with HIPAA regulations. Provide specialized care for residents with acute conditions such as pain management and infectious diseases. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services. Support residents across the lifespan—from geriatric dementia and Alzheimer's care—delivering tailored interventions in diverse environments. Skills Proven experience working with electronic medical records (PCC) for efficient documentation. Strong knowledge of physiology, anatomy, medical terminology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques. Skilled in resident assessment techniques including vital signs measurement, physical examinations, specimen collection/processing, and lab specimen handling. Expertise in administering injections, catheter care, and airway management. Experience caring for diverse populations such as seniors in geriatrics or memory/dementia care. Ability to perform triage effectively in urgent or emergency situations. Knowledge of infection control practices and sterile processing procedures to ensure resident safety at all times. Excellent communication skills for collaborating with healthcare teams and educating residents about their treatment plans while maintaining HIPAA compliance.
