Warren General Hospital is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Behavioral Health Unit on the night shift . The Behavioral Health LPN provides safe, therapeutic, and patient-centered nursing care to individuals experiencing mental health and behavioral health conditions. The LPN works as a member of the healthcare team, using focused assessment, sound nursing judgment, and established care plans and orders while collaborating with the RN and other members of the interdisciplinary team.
Key Responsibilities:
Provide direct nursing care to behavioral health patients in accordance with hospital policies, standards of care, regulatory requirements, and the Pennsylvania Practical Nurse scope of practice. Perform focused nursing assessments and ongoing observations; recognize and promptly report changes in patient condition, safety concerns, or needs that exceed the LPN's scope, knowledge, skill, or ability. Administer medications and carry out ordered therapeutic treatments within the LPN's scope of practice, and monitor and report therapeutic effects, adverse reactions, and changes in patient response. Implement and contribute to the evaluation of individualized nursing care plans in collaboration with the RN and healthcare team. Maintain a safe and therapeutic environment, including use of approved de-escalation and crisis-intervention techniques consistent with hospital policy and training. Accurately document patient care, observations, medication administration, interventions, and patient responses in the electronic medical record. Communicate patient status, observations, and changes in condition to the RN, provider, and other appropriate healthcare team members. Participate in treatment team activities and collaborate with physicians, RNs, therapists, social workers, and other healthcare professionals to support patient recovery. Reinforce patient and family education related to treatment plans, medications, coping strategies, and other care instructions as directed and within the LPN scope of practice. Uphold patient confidentiality and demonstrate respect, dignity, empathy, and professionalism at all times. Support night-shift unit operations, including patient monitoring, admissions, safety checks, and emergency response as assigned and within scope. Qualifications Current Pennsylvania LPN license in good standing. Graduate of an approved practical nursing program. Previous experience in behavioral health, mental health, or psychiatric nursing preferred; new graduates are encouraged to apply. Current BLS/CPR certification. Strong communication, observation, critical-thinking, and interpersonal skills. Ability to remain calm, professional, and compassionate in challenging situations. Ability to work collaboratively with RNs and other members of the healthcare team and to seek guidance when patient needs exceed the LPN's scope of practice or competency.
