Join The Rivers at Maryland Farms in Brentwood, TN! We are looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our team on day shift, 7:00 AM-7:00 PM . Schedule Full-Time Day Shift 7:00 AM - 7:00 PM 12-hour shifts Work alongside a dedicated team of nurses, caregivers, and community leadership $2,500 Sign-On Bonus Join our team and receive a $2,500 Sign-On Bonus! $1,250 paid after hire $1,250 paid after 90 days of employment Sign-on bonus eligibility and additional details will be discussed during the hiring process. What You'll Do As an LPN at The Rivers at Maryland Farms, you'll play an important role in providing exceptional care and creating a safe, comfortable environment for our residents. Provide direct resident care while following individualized plans of care. Administer medications in accordance with physician orders, state regulations, standards of practice, and community policies. Perform resident assessments as required by state regulations and community policies. Monitor residents for changes in condition and promptly communicate concerns to appropriate leadership. Document resident observations, clinical procedures, communications, and care provided during your shift. Maintain and update individualized resident care plans. Collaborate with our nursing, caregiving, and leadership teams to ensure residents receive excellent care. Communicate effectively with residents, families, physicians, and other members of the care team. Provide guidance and direction to other associates as appropriate.
