Job Description

Job Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in various healthcare settings, including inpatient care, assisted living, and long-term care facilities. As an LPN, you will play a crucial role in providing high-quality patient care, assisting with discharge planning, and ensuring that patients receive the best possible support during their recovery. Duties Provide direct patient care under the supervision of registered nurses and physicians. Administer medications and treatments as prescribed by healthcare providers. Monitor patients' vital signs and report any significant changes to the nursing staff. Assist with discharge planning and ensure that patients understand their care instructions. Maintain accurate patient records using electronic health record systems such as Epic and eClinicalWorks. Collaborate with interdisciplinary teams to develop and implement patient care plans. Educate patients and their families about health management, including the use of Durable Medical Equipment (DME). Support occupational health initiatives within the facility to promote overall wellness. Qualifications Valid LPN license in the state of practice. Experience in inpatient settings or long-term care preferred. Familiarity with electronic health record systems such as Epic or eClinicalWorks is a plus. Strong communication skills and ability to work effectively within a team environment. Compassionate approach to patient care with a focus on quality service delivery. Knowledge of discharge planning processes and patient education techniques is desirable.
