Job Description

Job Summary We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to provide PRN (as-needed) home health visits, delivering compassionate patient care in a variety of community settings. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation, ensuring accurate documentation in electronic health records (EHR/EHR systems). Administer medications, including injections and IV infusions, following strict aseptic techniques and safety protocols. Provide wound care, catheterization, tube feeding management, and ventilator support as needed, adhering to infection control standards. Educate patients and caregivers on disease management, medication usage, mobility assistance, and safety measures tailored to individual needs such as dementia care or Alzheimer's support. Collaborate with interdisciplinary teams to develop and update personalized care plans that meet long-term care regulations and patient goals. Maintain compliance with HIPAA privacy policies while handling sensitive medical documentation and medical records. Utilize EMR/EHR systems like Epic or Cerner for accurate data entry, intake processing, and clinical documentation. Respond promptly to urgent triage calls or emergent situations, providing critical guidance based on acute pain management or emergency medicine protocols. Support patients with complex needs such as infectious disease care or post-acute recovery following hospital or ICU stays. Communicate effectively with patients' families to ensure understanding of care plans and foster caregiver engagement. Experience Valid LVN license with current state licensure required. Prior experience working in home health settings or community-based care preferred; experience with skilled nursing facilities or assisted living environments is a plus. Demonstrated proficiency in medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation standards. Critical care experience such as ICU or Level I Trauma Center exposure highly desirable; experience with acute care or emergency medicine is beneficial. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or similar platforms. Knowledge of long-term care regulations, geriatrics principles, dementia/dementia care strategies, and hospice support is advantageous. Strong assessment skills including vital signs monitoring, physical examinations, airway management, venipuncture (phlebotomy), catheter care, and basic life support (BLS/CPR). Experience working with individuals with disabilities or developmental delays enhances your ability to deliver comprehensive patient-centered care. Excellent communication skills for effective caregiver interaction and client education across diverse populations including pediatric to geriatric patients.
