The ideal candidate will possess a strong foundation in patient assessment, medication administration, and medical documentation, with experience working with diverse populations including seniors, individuals with disabilities, and those requiring home health services. As an LVN, you will play a vital role in delivering high-quality patient care, supporting treatment plans, and ensuring compliance with healthcare regulations. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Cradlemrx, Epic or Cerner. Skilled assessment, skilled observation, skilled intervention and skilled teaching. Complete evaluation tasks, including reviewing medication and vital signs. Administer medications and injections accurately while adhering to sterile techniques and safety protocols. Assist with complex procedures such as IV infusion, catheterization, tube feeding, and wound dressing changes in accordance with established clinical guidelines. Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients. Maintain detailed medical records and documentation compliant with HIPAA regulations and ICD coding standards (ICD-10). Support discharge planning, patient education, and health coaching to promote recovery and well-being across outpatient, inpatient, home care settings. Requirements Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). • Reliable transportation with current valid auto insurance. • Current

screen. • Must have excellent interpersonal skills, strong organizational skills and be detail oriented. Proven experience in diverse healthcare environments including hospital medicine, medical-surgical units, home health, hospice care, or long-term care facilities. Demonstrated proficiency with EMR/EHR systems such as Cradlemrx, Epic, Cerner, Athenahealth, or eClinicalWorks. Strong knowledge of physiology, anatomy, pharmacology related to medication administration and pain management. Experience working with individuals with developmental disabilities or special needs populations such as memory care residents or pediatric patients. Ability to perform advanced skills including phlebotomy, venipuncture, sterile processing techniques, and specimen collection/processing. Excellent communication skills coupled with critical thinking abilities to perform triage assessments and respond effectively in urgent care scenarios. Pay:
$35.00 per visit. This is a per visit position @ 20 to 40 hours per week from Monday to Friday.
