The ideal candidate will possess a strong foundation in patient assessment, medication administration, and medical documentation, with experience working with diverse populations including seniors, individuals with disabilities, and those requiring home health services. As an LVN, you will play a vital role in delivering high-quality patient care, supporting treatment plans, and ensuring compliance with healthcare regulations. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Cradlemrx, Epic or Cerner. Skilled assessment, skilled observation, skilled intervention and skilled teaching. Complete evaluation tasks, including reviewing medication and vital signs. Administer medications and injections accurately while adhering to sterile techniques and safety protocols. Assist with complex procedures such as IV infusion, catheterization, tube feeding, and wound dressing changes in accordance with established clinical guidelines. Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients. Maintain detailed medical records and documentation compliant with HIPAA regulations and ICD coding standards (ICD-10). Support discharge planning, patient education, and health coaching to promote recovery and well-being across outpatient, inpatient, home care settings.
