•We are seeking experienced Licensed Practical Nurses (LPNs) - Hospice for one of our clients, a leading home healthcare service provider.

Pay Rate:

$45/hr

Position Requirements:

Current and active LPN license in Vermont and New Hampshire, or an active compact license. Graduation from an accredited/approved Practical Nursing program. Minimum 2 years of nursing experience . Experience in hospice, home health, medical-surgical nursing, or a related setting preferred. Current

BLS/BCLS

certification required.

Responsibilities:

Provide compassionate nursing care to hospice patients in their homes. Make home visits under the direction of the RN Case Manager. Monitor patients' physical and emotional condition and report changes promptly. Assist with end-of-life care and comfort measures. Administer medications and treatments as directed by the care plan. Educate patients and families regarding care, medications, and symptom management.
