Day & Night Shifts Position:
Licensed Practical Nurse (LPN) Position Summary We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide quality nursing care to veterans in a Skilled Nursing and Assisted Living environment. The LPN will provide direct resident care, administer medications and treatments, monitor resident conditions, and work closely with RNs, CNAs, providers, residents, and families. Key Responsibilities Provide direct nursing care in accordance with resident care plans, facility policies, and applicable nursing standards. Administer medications and treatments safely and accurately within the LPN scope of practice. Monitor and document vital signs, symptoms, intake/output, and changes in resident condition . Perform routine nursing assessments and promptly report significant changes to the RN, provider, or appropriate healthcare professional. Assist with wound care, treatments, specimen collection, and other nursing procedures as assigned. Supervise and collaborate with CNAs and other nursing staff to ensure residents receive safe and appropriate care. Respond promptly to resident needs and call lights. Maintain accurate and timely clinical documentation and resident records . Follow proper infection-control, medication-safety, and facility procedures . Participate in care planning, care conferences, and interdisciplinary communication as required. Respond appropriately to emergencies and assist with CPR, first aid, evacuation, and other emergency procedures as trained. Communicate professionally and compassionately with residents, families, and members of the healthcare team. Promote resident dignity, privacy, independence, safety, comfort, and quality of life . Ideal Candidate Current and active LPN/LVN license . Previous experience in Skilled Nursing, Long-Term Care, Assisted Living, or Veterans' Care preferred. Strong communication, clinical, organizational, and documentation skills. Compassionate and respectful approach to caring for veterans and older adults. Reliable, team-oriented, and able to work effectively in a fast-paced healthcare environment. Comfortable working 12-hour day or night shifts . Pay:
