Job Description

Job Summary We are seeking a compassionate and experienced Hospice & Palliative Care Medical Director to provide clinical and administrative leadership for a growing hospice and palliative care program. This role focuses on delivering exceptional, patient-centered care, leading interdisciplinary teams, supporting quality initiatives, and partnering with providers to enhance hospice and palliative services. Key Responsibilities Provide medical leadership and oversight for hospice and palliative care services. Determine patient eligibility and certify/re-certify hospice and palliative care admissions. Collaborate with interdisciplinary teams to develop and oversee individualized care plans. Consult with attending physicians and specialists regarding patient care and symptom management. Conduct patient evaluations in home, outpatient, and inpatient settings as needed. Lead quality improvement initiatives and participate in clinical leadership committees. Educate clinical staff, physicians, and community partners on hospice and palliative care best practices. Support the development and growth of home-based palliative care services. Provide guidance on pain management, symptom control, and end-of-life care. Build relationships with referral sources and healthcare providers to expand program access. Qualifications MD or DO with Board Certification/Board Eligibility in Hospice & Palliative Medicine . Board Certified in Internal Medicine or Family Medicine . Completion of a Palliative Medicine Fellowship preferred. Active, unrestricted New York State Medical License and DEA Registration . 1 2+ years of physician experience, including hospice, palliative care, pain management, or symptom management. Experience working within an interdisciplinary hospice or palliative care team. Excellent leadership, communication, and collaboration skills.
