Job Description

Compensation Nurse Practitioner:
$68-$78 per hour
Physician Assistant:
$55-$66 per hour Join Our Community-Focused Primary Care Team Serenity Care Health Group is seeking a compassionate and clinically strong Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient primary care team in Rowland Heights. As an FQHC Look-Alike (FQHC-LAL), we are committed to providing accessible, culturally responsive, patient-centered healthcare to the communities we serve. Our diverse patient population includes many Mandarin- and Cantonese-speaking patients and families, making language skills and cultural understanding especially valuable in building trust and continuity of care. Our care model emphasizes collaboration, clinical quality, prevention, and whole-person care. Providers work closely with physicians, clinical staff, and other members of the care team. Our clinic also offers acupuncture services, providing additional opportunities for collaboration and a broader approach to patient health and wellness. This is an excellent opportunity for an NP or PA who values community-based medicine, meaningful patient relationships, teamwork, and the opportunity to make a lasting impact on the health of the patients we serve. What You'll Do Conduct comprehensive patient assessments, including medical histories, physical examinations, and appropriate clinical evaluations. Diagnose and manage acute and chronic medical conditions. Develop and implement individualized, evidence-based treatment and care plans. Order, review, and interpret appropriate laboratory studies and diagnostic testing. Prescribe medications and treatments in accordance with applicable California laws and scope of practice. Provide preventive care, health screenings, patient education, and counseling on wellness, lifestyle, and disease management. Support patients in managing chronic conditions and achieving their health goals. Provide culturally responsive care and communicate effectively with patients and families. Build continuity of care and strong, long-term patient relationships. Collaborate closely with physicians, clinical staff, and other members of our multidisciplinary healthcare team. Provide wound assessment and routine wound management when clinically appropriate and within scope of practice. Maintain accurate and timely documentation of patient encounters using eClinicalWorks (eCW). Participate in quality improvement, patient satisfaction, and other initiatives that support high-quality community healthcare. Follow applicable clinical, infection-control, safety, and regulatory standards. What We're Looking For We are looking for a provider who combines strong clinical judgment with compassion, professionalism, cultural awareness, and a genuine interest in caring for a diverse community. Required Active and unrestricted California Nurse Practitioner or Physician Assistant license. Current applicable national certification (NP certification or PA-C). Fluency in Mandarin and/or Cantonese, with the ability to communicate effectively with our Chinese-speaking patients and families. Strong clinical assessment and decision-making skills. Excellent communication and interpersonal skills. Ability to provide compassionate, culturally responsive care to a diverse patient population. Commitment to professionalism, reliability, teamwork, and patient-centered care. Ability to work collaboratively as part of a multidisciplinary healthcare team. Willingness to work on-site in Rowland Heights, California. Preferred 2+ years of clinical experience. Experience in primary care, family medicine, internal medicine, community health, or another outpatient setting. Experience with eClinicalWorks (eCW) is a plus. Mondays:
8:30 AM-1:30
PM Thursdays:
