  • Wound Care Nurse Practitioner Opportunity ## Overview Integrated Care LLC is seeking a compassionate, highly skilled Board-Certified Nurse Practitioner with wound care expertise to join our growing mobile and telehealth-based practice.
This role offers a unique opportunity to deliver specialized wound care directly to patients who need it most — including homebound adults, geriatric patients, post-surgical patients, and individuals with complex medical needs. As a Wound Care NP with Integrated Care, you will provide expert wound assessment and treatment in home settings, care homes, group homes, and via telehealth, helping patients heal faster, avoid infections and amputations, and reduce unnecessary hospitalizations. This is an ideal role for a provider who values autonomy, flexibility, and meaningful impact in the community. ## What You'll Do
  • Perform comprehensive wound assessments, including wound measurement, staging, tissue evaluation, and vascular screening
  • Diagnose and manage acute and chronic wounds, including pressure injuries, diabetic foot ulcers, venous and arterial ulcers, surgical wounds, skin tears, and traumatic wounds
  • Develop and implement individualized wound treatment plans, including advanced dressing selection and modality recommendations
  • Perform procedures including sharp/conservative debridement, wound cultures, suturing, injections, and application of advanced wound care products
  • Order and interpret labs, imaging, and diagnostic tests relevant to wound healing (e.g., vascular studies, nutritional markers, infection workups)
  • Manage underlying conditions that impact healing, such as diabetes, peripheral vascular disease, edema, and malnutrition
  • Provide urgent wound-related care in the home to prevent ER visits and hospital readmissions
  • Conduct telehealth wound consultations and follow-ups while maintaining HIPAA compliance
  • Document thoroughly using EMR systems (e.g., Tebra/Kareo, Epic, Cerner, eClinicalWorks), including wound photography and measurement tracking
  • Collaborate with interdisciplinary teams including home health nurses, case managers, caregivers, podiatrists, and vascular specialists
  • Support chronic care management (CCM) and care coordination for patients with non-healing wounds
  • Educate patients and caregivers on wound care techniques, offloading, pressure injury prevention, and nutrition for healing ## What We're Looking For
  • Board-Certified Nurse Practitioner (AANP or ANCC) — FNP, AGNP, or AGACNP
  • Wound care certification (CWCN, CWS, CWSP, WCC, or equivalent) or substantial clinical wound care experience
  • Active and unrestricted state license
  • Strong clinical experience in wound care, home health, hospital-based wound programs, or wound clinics
  • Skilled in debridement, dressing selection, compression therapy, and negative pressure wound therapy (NPWT) management
  • Comfort managing complex, high-acuity, and underserved populations
  • Experience with geriatric care and chronic disease management preferred
  • Proficiency with EMR/EHR systems and medical documentation
  • Ability to work independently in mobile and home-based environments
  • Strong communication and clinical decision-making skills ## Preferred Experience (Bonus, Not Required)
  • Advanced certifications such as CWS, CWSP, or CWOCN
  • Experience with hyperbaric medicine, skin substitutes/cellular tissue products, or ostomy care
  • Home-based primary care or mobile medicine background
  • Knowledge of
CPT/ICD-10
coding for wound care and value-based care models (CCM)
  • Experience with hospice, palliative care, or complex care coordination
