Job Description

Matrix Medical Network is seeking a Nurse Practitioner to join our Healthcare & Medical Services team on a PRN basis. In this role, you will perform in-home health assessments, support quality assurance initiatives, and contribute to health management programs for insurance plans and government agencies. You'll use your clinical expertise to improve patient outcomes while helping reduce healthcare costs. Responsibilities Conduct comprehensive in-home health assessments, including history, physical exam, and functional evaluations. Review and reconcile medications, identifying potential interactions, adherence issues, and optimization opportunities. Identify and document chronic conditions, acute concerns, and care gaps using evidence-based guidelines. Educate patients and caregivers on disease management, preventive care, medications, and lifestyle modifications. Order, review, and interpret appropriate diagnostic tests within scope of practice and organizational protocols. Document all encounters accurately and thoroughly in electronic health records (EHR) in a timely manner. Collaborate with Matrix Medical Network clinical teams, primary care providers, and health plan partners to support coordinated care. Escalate urgent findings and safety concerns according to established protocols and state practice regulations. Adhere to infection control, safety, and privacy standards while working in patients' homes and community settings. Manage a flexible PRN schedule, traveling to assigned visits and ensuring punctual, reliable patient coverage. Participate in quality improvement, training, and process-enhancement initiatives as requested by leadership.
