Family Nurse Practitioner (APRN, FNP-C) - Primary Care Grace Primary Care PLLC

• Full-time (4-5 day schedule available) About Grace Primary Care Grace Primary Care is an independent, provider-owned family practice in Crestwood, Kentucky, 20 minutes from Louisville. Our team of three providers (a family medicine DO and two APRNs) and four clinical/front-office staff serves a loyal, multi-generational patient panel. We take a whole-person, relational approach and offer in-house preventive and diagnostic services including annual wellness visits, cardiovascular and autonomic function testing, DOT and sports physicals, remote patient monitoring, and osteopathic manipulative treatment. We accept Medicare, Medicaid MCOs, and all major commercial plans. The Role We are hiring a Family Nurse Practitioner to take over an established patient panel from a departing APRN. This is a see-your-own-patients role with a built-in schedule from day one. A typical clinic day runs 9:00 AM-5:00 PM and includes: 30-minute annual/preventive exams (Medicare AWVs and commercial well visits), about 40% of visits 15-minute chronic-care follow-ups (hypertension, diabetes, hyperlipidemia, thyroid, pain/spine, headache, vertigo), about 50% of visits 30-minute new-patient visits, same-day problem visits, and hospital/transitional-care follow-ups Ordering in-office cardiovascular/autonomic testing and reviewing results with patients Chronic care management and remote patient monitoring oversight Current template: 4 clinic days/week, 10-20 scheduled patients/day with room to grow. No hospital rounding, no call, no weekends. What You Will Do Provide comprehensive primary care to adults and adolescents: preventive, acute, and chronic disease management Perform annual wellness visits and well exams, including Medicare AWV documentation and preventive screenings Manage chronic conditions with evidence-based treatment plans and appropriate specialist referral Order and interpret labs, ECGs, and in-office diagnostics; document diagnoses accurately and completely Complete DOT physicals (FMCSA certification preferred or obtained within 6 months), sports and school physicals Chart in the practice EHR/PM system (MDOfficeManager) with timely, complete documentation and coding Collaborate with the supervising physician, fellow APRN, CMAs, and front desk on care coordination, prior authorizations, and refills Participate in quality and care-management programs (AWV recall, RPM, chronic care/APCM enrollment) Required Qualifications MSN or DNP from an accredited program Current Kentucky APRN license (or eligible) with prescriptive authority (CAPA-NS and CAPA-CS) National board certification as a Family Nurse Practitioner (FNP-C or FNP-BC) Active DEA registration or eligibility; BLS certification Eligible to enroll with Medicare, Kentucky Medicaid, and commercial payers Preferred Qualifications 2+ years outpatient primary care experience as an

NP FMCSA

National Registry medical examiner certification (DOT physicals) Experience with Medicare AWVs, transitional care management, and chronic care/remote monitoring programs Interest or training in integrative, functional, or lifestyle medicine Background in occupational health, neurology, or musculoskeletal/spine care Familiarity with MDOfficeManager or similar EHR/PM systems Schedule Monday-Friday clinic hours, 4-5 day schedule available; approximately 9:00 AM-5:00 PM patient hours; no nights, weekends, call, or inpatient duties.
