On the road About This Opportunity HomeMD is connecting with Nurse Practitioners in Genesee County and nearby communities . Our providers bring primary care directly to seniors and other homebound patients who may have difficulty accessing traditional clinic-based care. Visits take place in patients' homes and residential care environments, allowing providers to develop meaningful relationships with patients while supporting continuity of care. What You'll Do As a mobile Nurse Practitioner with HomeMD, you may provide care to patients in private residences, assisted living communities, adult foster care settings, and other residential environments.
Your work may include:
Performing comprehensive in-home patient evaluations Completing wellness and follow-up visits Assessing changes in patient health and determining appropriate interventions Managing chronic conditions such as diabetes, cardiovascular disease, hypertension, and cognitive conditions Reviewing and managing patient medications Educating patients, families, and caregivers regarding treatment plans and medication use Ordering and reviewing laboratory testing, imaging, and other diagnostic services Conducting preventive screenings and health assessments Evaluating fall risks and other safety concerns Coordinating basic wound care needs and appropriate referrals Responding to changes in patient condition through in-person or remote follow-up when appropriate Collaborating with physicians, pharmacies, caregivers, facility staff, and other healthcare professionals Maintaining complete, accurate, and timely clinical documentation Mobile Care With a Purpose HomeMD's care model is designed to make healthcare more accessible for senior and homebound populations. By providing care where patients live, our Nurse Practitioners help: Identify health concerns before they become more serious Improve management of chronic medical conditions Reduce barriers associated with transportation and mobility Promote consistent follow-up and continuity of care Support patients, caregivers, and families navigating complex healthcare needs Your Service Area Providers typically serve patients within approximately a 45-mile radius of their home location . Occasional travel outside the usual service area may be needed based on patient coverage and operational needs. For this pipeline, we are especially interested in connecting with providers who can support Genesee County and surrounding communities . Support Behind You Although our Nurse Practitioners work independently in the field, they are supported by a centralized team that assists with scheduling, logistics, and administrative coordination. This structure allows providers to dedicate more of their time and attention to clinical care and their patients. What We're Looking For Qualified candidates should have: An active Michigan Nurse Practitioner license Master's or Doctoral degree in Nursing Current applicable board certification Strong independent clinical decision-making skills Effective communication and patient education skills Ability and willingness to provide care in a mobile, field-based environment Experience in geriatrics, primary care, home-based care, chronic disease management, palliative care, hospice, or complex patient care is preferred.

Compensation & Benefits Compensation:
