As a key member of our clinical staff, the FNP/PA will provide comprehensive primary care services across the lifespan in a team-based environment. Your daily work will include health maintenance, health promotion, and the evaluation and management of both acute and chronic conditions. We are especially interested in candidates who bring early to intermediate experience with Value‑Based Care (VBC) and Full‑Risk care models , or those who are eager to grow in this space. Our practice is designed for clinicians who want to focus on patient outcomes, continuity, and long‑term wellness.
