There are senior living communities throughout Greater Minnesota where access to consistent primary care isn't as simple as scheduling an appointment and driving ten minutes to a clinic. There are older adults in and around Willmar who need a provider willing to come to them. We're looking for that provider. Yes, this position requires travel. But at the end of that drive are seniors, nurses, caregivers, and families who need someone they can know, trust, and count on . As a community-based Nurse Practitioner with Twin Cities Physicians, you'll bring proactive primary care directly into senior living communities, helping improve access, continuity of care, and quality of life while reducing unnecessary hospitalizations . Rural Healthcare Needs Providers Willing to Show Up Senior living communities are caring for increasingly complex residents. Transportation can be difficult, chronic conditions require consistent management, and fragmented healthcare can quickly lead to unnecessary emergency room visits and hospitalizations. That's where you come in. You'll provide on-site primary care in assisted living, memory care, independent living, and skilled nursing communities throughout Willmar and the surrounding area. Instead of asking medically complex seniors to continually navigate the healthcare system, you'll bring healthcare to them . Over time, you'll become more than another provider making rounds. Supportive, physician-led leadership team . Practice Medicine Where You're Needed You'll develop lasting relationships with the residents and communities you serve. You'll learn your patients' histories, recognize subtle changes in condition, manage chronic illnesses, coordinate care after hospitalizations, and collaborate with the nurses and caregivers who see them every day. You may identify a change before it becomes an emergency. You may help a resident avoid an unnecessary trip to the hospital. What You'll Be Doing Provide comprehensive primary care throughout assisted living, memory care, independent living, and skilled nursing communities . Travel throughout Willmar and surrounding communities to bring medical care directly to patients. Diagnose and manage acute illnesses and chronic medical conditions. Perform comprehensive assessments, annual wellness visits, and routine follow-up care. Develop individualized treatment plans focused on long-term outcomes. Monitor changes in patient health over time . Coordinate care following hospitalizations and specialty consultations to help reduce avoidable readmissions . Collaborate with physicians, nurses, therapists, pharmacists, social workers, caregivers, and senior living teams. Educate patients and families while serving as a trusted clinical resource . Improve quality measures, patient satisfaction, and continuity of care . Clinical Backgrounds That Transition Well We welcome Nurse Practitioners with experience in: Primary Care. Family Practice. Internal Medicine. Geriatrics. Long-Term Care. Skilled Nursing. Home Health. Hospice & Palliative Care. Hospital Medicine. Community-Based Medicine. You don't need to have spent your career in rural healthcare, but you should understand why bringing care to patients who may otherwise struggle to access it matters . What We're Looking For Required Qualifications Active Minnesota Nurse Practitioner (NP) license. Strong assessment, diagnostic, and clinical decision-making skills . Ability to practice independently while collaborating within a physician-led team. Strong communication and care coordination skills. Passion for caring for older adults. Reliable transportation and willingness to travel throughout Willmar and surrounding communities . Preferred Qualifications Experience managing medically complex seniors . Chronic disease management experience. Primary care, geriatric, or community-based clinical experience. Assisted living, memory care, or skilled nursing experience.
