You'll provide on-site primary care to seniors living in assisted living, memory care, independent living, and skilled nursing communities throughout Willmar and the surrounding area. Instead of waiting for patients to come to a clinic, you'll bring care directly to where they live. More importantly, you'll have the opportunity to know the patients you're caring for . You'll follow their health over time, understand their history, recognize changes, build relationships with their families and care teams, and become a provider communities know and trust. You'll develop a panel of patients and become an ongoing clinical resource for the senior living teams you serve. That continuity matters. When you know someone's baseline, you're better positioned to recognize when something changes. When community nurses know and trust you, communication becomes easier. When families know there is a consistent provider involved, they have someone they can turn to when questions arise. You'll have the autonomy to assess, diagnose, treat, manage chronic conditions, and develop individualized care plans , while still having the support and collaboration of a physician-led medical group behind you. You'll round within senior living communities throughout Willmar and the surrounding area , working closely with the nurses and caregivers who interact with your patients every day. You'll manage chronic conditions, address acute concerns, complete wellness visits, coordinate transitions after hospitalization, review medications, and monitor changes in health over time. Because you're seeing patients where they live, you also gain a perspective that's difficult to get during a traditional clinic appointment. You can better understand their environment, functional status, support system, and the challenges affecting their health. You're not just treating today's problem. You're managing the whole patient. What You'll Be Doing Provide comprehensive on-site primary care within assisted living, memory care, independent living, and skilled nursing communities Manage your own panel of older adult and medically complex patients Diagnose and treat acute illnesses and chronic medical conditions Perform comprehensive assessments, annual wellness visits , and routine follow-up care Develop individualized treatment plans focused on long-term health and quality of life Monitor changes in patient health over time Manage medications and chronic disease treatment plans Coordinate care following hospitalizations and specialty consultations Help identify clinical concerns early and reduce avoidable hospitalizations and readmissions Collaborate with physicians, nurses, therapists, pharmacists, social workers, caregivers, and senior living teams Educate patients and families while serving as a trusted clinical resource Support quality measures, patient satisfaction, and continuity of care Clinical Backgrounds That Transition Well We welcome Nurse Practitioners with experience in: Primary Care Family Practice Internal Medicine Geriatrics Long-Term Care Skilled Nursing Home Health Hospice & Palliative Care Hospital Medicine Community-Based Medicine You don't need to come directly from senior living. What We're Looking For Required Qualifications Active Minnesota Nurse Practitioner (NP) license Strong assessment, diagnostic, and clinical decision-making skills Ability to practice independently while collaborating within a physician-led team Strong communication and care coordination skills Passion for caring for older adults Reliable transportation and willingness to travel throughout Willmar and surrounding communities Preferred Qualifications Experience managing medically complex seniors Chronic disease management experience Primary care, geriatric, or community-based clinical experience Assisted living, memory care, or skilled nursing experience Wound Care Certification (WCC) is a plus A Different Kind of Primary Care Career This role does require travel between communities, but it also offers something many traditional clinical environments struggle to provide: autonomy, continuity, flexibility, and the opportunity to build genuine relationships with the people you care for .
