Job Summary:
We are seeking a friendly and compassionate Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive primary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. Responsibilities:
Provide primary care services to residents in post-acute care settings Evaluate, treat, and manage patients experiencing acute or worsening medical conditions requiring timely intervention Perform physical exams and medical history assessments Prescribe medications and monitor treatment effectiveness Develop and implement individualized treatment plans Collaborate with interdisciplinary teams Educate patients, families, and facility staff on respiratory care and treatment plans Monitor patient progress and adjust care plans as needed Maintain accurate and timely EMR documentation Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events
Required Qualifications:
Preferred a minimum of 6 months of experience in post-acute care setting Preferred experience: training provided Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required Master's Degree in Nursing from an accredited Nurse Practitioner program or Master's Degree from an accredited Physician Assistant program required.
CPR BCLS/BLS
certified Strong communication, organizational, and teamwork skills Ability to work independently in the clinical environment
