Bring your expertise as a Nurse Practitioner to a flexible, patient-focused role providing home health risk assessments. In this per diem opportunity, you'll utilize your clinical skills by visiting Medicare Advantage plan members across Pike County, ensuring high-quality healthcare in the comfort of their homes. Assessments may include annual health reviews or post-discharge evaluations, typically taking about 45 minutes each. With a range of visit locationsincluding Waverly, Pee Pee Township, and Seal Townshipyou'll have the chance to serve your community while enjoying dynamic, rewarding work.
Schedule:
Flexible shifts available any day between Monday and Sunday Empowerment to create your own schedule Minimum weekly or bi-weekly hourly commitment
Responsibilities:
Carry out comprehensive health risk assessments in home settings Review patients current and previous medical history, conditions, and medication use Offer education on diagnoses and prescribed medications to support patient understanding Identify gaps in care and recommend steps for improvement No requirement for creating treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner license in Ohio Current CPR certification (BLS) Board certification through AANP or ANCC At least 6 months of recent NP experience preferred Master of Science in Nursing (MSN) from an accredited program This position offers $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
