Providers work as part of an interdisciplinary team delivering longitudinal, facility-based care focused on improving outcomes for medically complex seniors. Key Responsibilities Conduct comprehensive patient assessments Manage acute illnesses and chronic disease exacerbations Support patients during short-term rehabilitation stays Ensure accurate documentation and ICD-10 coding Collaborate with interdisciplinary care teams Conduct advanced illness and advance care planning discussions Educate and counsel patients and caregivers Coordinate care and treatment planning across care settings

Required Qualifications NP:

MSN or DNP degree with AANP or ANCC certification

PA:

Graduate of an accredited Physician Assistant program with NCCPA certification Active, unrestricted state license (or ability to obtain) DEA license/prescriptive authority (or ability to obtain) Ability to obtain a collaborative practice agreement if required Valid driver's license and reliable transportation Ability to perform physical assessments and travel between patient care locations Preferred Qualifications 1+ years of clinical experience, preferably in long-term care, geriatrics, or senior living Experience caring for medically complex patients Knowledge of geriatrics, chronic illness, and end-of-life care discussions EMR proficiency and strong documentation skills Highlights Facility-based senior care role focused on high-risk patient populations Value-based care emphasizing quality outcomes rather than visit volume Strong interdisciplinary team support No call, weekends, or holidays Opportunity to build meaningful relationships with seniors and make a significant impact on patient outcomes through coordinated, longitudinal care.
