Job Description

TriValley Primary Care is looking for a full time, experienced CRNP for our busy family practice in Quakertown, PA. The Nurse Practitioner is responsible for providing direct medical care for acute and chronic conditions, and annual physical examinations in an office setting. Will see patients from pediatrics through geriatrics.

Participation in value-based care with staff support
Team-based care including a care manager and population health specialist
Opportunities for networking with peers within the group
Successful candidate has at least 2 years experience as CRNP delivering direct patient care, ideally in a primary care or similar setting.

Must have experience with an EMR and a commitment to building patient relationships. Must possess a current license in the state of Pennsylvania to practice as a Registered Nurse and as a Nurse Practitioner. Must be certified as a Nurse Practitioner by the American Nurses Credentialing Center. Full time position is based on a four-day work week and 33 patient-contact hours.
