In this vital role, you will provide high-quality, personalized care to clients in their homes or care facilities, promoting independence and well-being. Your dedication will help enhance the quality of life for individuals with various health conditions, including memory care, post-acute recovery, and long-term support. Duties Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Administer medications and monitor vital signs in accordance with care plans and medical directives Support clients with specialized needs including Alzheimer's, dementia, behavioral health, and memory care Proven experience as a CNA or caregiver in settings such as hospitals, nursing homes, assisted living facilities, or home care environments Ability to perform complex patient care tasks including phlebotomy, medication administration, and use of medical equipment like Hoyer lifts and catheters Experience working with individuals with disabilities or behavioral health challenges; expertise in dementia and Alzheimer's care preferred.
