Job Description

Heritage Health Care Center, part of HHP Lifeshare in Minster, Ohio, seeks a compassionate Certified Nursing Assistant to support individuals with developmental disabilities in residential and community settings. In this role, you will assist with daily living tasks, personal care, mobility, and health monitoring while promoting dignity, independence, and inclusion. CNAs collaborate closely with nursing and support teams, maintain accurate documentation, and follow individualized care plans. Responsibilities Provide direct personal care, including hygiene, bathing, dressing, and toileting Assist individuals with developmental disabilities with daily living and independence goals Monitor and report changes in health status, behavior, and safety concerns Measure and document vital signs and basic health observations per care plan Support mobility, transfers, and use of adaptive equipment using safe practices Engage residents in person-centered activities, social outings, and community integration Maintain accurate, timely documentation in line with facility and regulatory standards Follow care plans, infection control procedures, and HIPAA/privacy requirements Collaborate with nurses, therapists, and support staff to ensure quality care Promote a respectful, family-like environment aligned with organizational values Required Skills Patient care Activities of Daily Living (ADLs) assistance Vital signs monitoring Infection control Electronic health records (EHR) documentation CPR/BLSSafe patient handling and transfers Medication assistance (non-licensed tasks per policy) Behavioral support for developmental disabilities HIPAA compliance
