Job Description

Job Overview We are seeking a dedicated and compassionate Home Health Aide (HHA) / Certified Nursing Assistant (CNA) to join our team. This vital role involves providing high-quality, patient-centered care in clients' homes or assisted living facilities. The ideal candidate will possess excellent observational skills, a strong understanding of medical terminology, and experience working with individuals with disabilities or dementia. This position offers an opportunity to make a meaningful difference in the lives of those in need while working in a supportive and professional environment. Responsibilities Provide direct patient care by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems. Assist with medication administration following proper protocols and ensuring compliance with HIPAA regulations to maintain patient confidentiality. Support patients with memory care needs, including Alzheimer's and dementia care, ensuring their safety and comfort at all times. Utilize medical terminology appropriately when documenting patient information and communicating with healthcare teams. Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively. Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation to promote overall well-being. Collaborate with healthcare professionals to develop and implement individualized care plans tailored to each client's needs. Demonstrate compassion and patience while working with clients experiencing hospice or end-of-life care situations. Experience Prior experience in home health care or nursing home settings is highly preferred. Proven ability to work effectively with individuals who have developmental disabilities or cognitive impairments such as Alzheimer's disease or other forms of dementia. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Knowledge of medical procedures including vital signs monitoring, medication administration, catheter care, and hospice support. Experience providing long-term care or senior care in assisted living environments is advantageous. Strong understanding of HIPAA regulations related to patient privacy and confidentiality. Demonstrated caregiving skills in home & community settings with a focus on safety, compassion, and professionalism. Pay:
$19.00 - $25.00 per hour
