Job Overview We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Caregiver to join our dynamic healthcare team. In this vital role, you will provide exceptional patient-centered care to individuals across various settings, including assisted living facilities, nursing homes, and home healthcare environments. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility assistance using proper patient transfer methods and equipment like Hoyer lifts when necessary Monitor and observe patient conditions closely, recording vital signs and reporting any changes promptly to nursing staff Administer medications and perform catheter care following medical orders and home healthcare regulations Maintain accurate medical documentation within Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and patient confidentiality standards Support patients during end-of-life or hospice care with compassion, dignity, and respect for their wishes Assist with meal preparation, feeding, light cleaning, laundry, and overall home & community care tasks to promote a safe living environment Utilize effective communication skills to collaborate with multidisciplinary teams, patients, families, and caregivers to deliver personalized care Skills Strong knowledge of medical terminology, patient care procedures, and home health care assistance protocols Experience working with geriatric patients, individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease or memory care needs Proficiency in using EMR/EHR systems for accurate medical records management and documentation Ability to perform patient transfers safely using methods like Hoyer lifts or other assistive devices; capable of heavy lifting when required Skilled in vital signs measurement, medication administration, CPR/First Aid/Basic Life Support certification for emergency response readiness Familiarity with home health agency operations, hospice care standards, and home health regulations Excellent client communication skills coupled with compassionate caregiving abilities for senior care or patients with disabilities Knowledge of infection control practices including cleaning routines and maintaining a safe environment for both patients and staff Join us if you are passionate about providing empathetic caregiving that enhances quality of life.
