Job Description

Job Summary Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA), where you will play a vital role in delivering high-quality, patient-centered care to individuals across various settings. Your energetic and empathetic approach will support patients with their daily needs, promote independence, and enhance their overall well-being. This paid position offers an opportunity to make a meaningful difference in the lives of those requiring assistance with personal care, medical needs, and emotional support. Responsibilities Provide personal care assistance including bathing, dressing, grooming, and mobility support to patients and clients. Assist with Activities of Daily Living (ADLs) such as eating, toileting, and transferring using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following home healthcare regulations and medication administration protocols while maintaining patient confidentiality in compliance with HIPAA standards. Support patients with dementia or Alzheimer's care, memory care needs, and end-of-life care with patience and dignity. Assist with light housekeeping tasks including cleaning, laundry, and meal preparation to ensure a safe and comfortable environment. Document patient progress accurately, communicate effectively with healthcare teams, and ensure compliance with all safety standards such as first aid, CPR, and basic life support procedures. Qualifications Valid CNA certification or PCA certification required; experience working in assisted living or nursing home environments is highly preferred. Demonstrated experience working with individuals with developmental disabilities or geriatrics. Knowledge of medical terminology, catheter care, and patient transfer methods essential for effective caregiving. Proficiency in using EMR/EHR systems for documentation; familiarity with home health agency operations is a plus. Ability to perform heavy lifting safely and operate equipment such as Hoyer lifts or other mobility aids. Strong communication skills for client interaction and team collaboration; experience working within home health or hospice care settings is advantageous. Certification in first aid, CPR, and basic life support required; ongoing training in dementia care, hospice support, or Alzheimer's care is beneficial.
