Overview Join our dedicated healthcare team as a PCA/CNA, where your compassionate care and attention to detail will make a meaningful difference in the lives of our patients. In this vital role, you will provide personalized assistance to individuals across various care settings, including assisted living, nursing homes, and home healthcare environments. Your expertise in patient observation, medication administration, and supporting activities of daily living (ADLs) will ensure our clients receive the highest quality of care. Responsibilities Observe and monitor patients' conditions regularly, documenting changes accurately in electronic health record (EHR) systems while maintaining strict patient confidentiality in accordance with HIPAA regulations Assist clients with activities of daily living such as bathing, dressing, grooming, toileting, and mobility using proper patient transfer methods and equipment like Hoyer lifts or gait belts Administer medications safely following medical terminology guidelines and home healthcare regulations under supervision or as authorized Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to track health status effectively Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and end-of-life support with patience and compassion Maintain a clean and safe environment by assisting with laundry, cleaning tasks, meal preparation, and ensuring proper infection control practices are followed Respond promptly to patient calls or emergencies using first aid skills and basic life support (BLS) techniques when necessary Qualifications Certified Nursing Assistant (CNA) license or Personal Care Assistant (PCA) certification required; relevant experience preferred Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly desirable Knowledge of medical terminology and medical documentation procedures essential for accurate record keeping Familiarity with EMR/EHR systems for charting patient information efficiently Experience working with individuals with disabilities or developmental challenges is a plus Ability to perform heavy lifting safely and use patient transfer equipment effectively Valid CPR and first aid certifications; ongoing training encouraged to stay current with best practices Strong communication skills for engaging with patients, families, and healthcare team members effectively Join us in delivering compassionate care that enhances the quality of life for our clients.
