Eligibility Criteria:

Minimum 2 year of Long-term care / Rehab experience. _____________________________________________________________________

Requirements:

Resume (Minimum 2 year of long-term care, Rehabilitation experience). Skills checklist Basic Life Support

• BLS Certification (must be from AHA
• American Heart Association).

2 Supervisory Reference Forms, taken within the last two years Driver's license Covid Card Flu shot.
