During assigned on-call hours, you will be the primary contact for caregiver questions, call-outs, urgent staffing issues, and client or caregiver concerns. If a caregiver calls out, you will work to find replacement coverage. If another qualified caregiver cannot be found, you must be available to personally provide the client shift. Routine scheduling is handled separately and is not the responsibility of this role. Pay & Benefits $18/hour during scheduled on-call coordination hours $25/hour when personally providing backup client care Weekly pay Paid orientation and shadow shifts Specialized dementia training Assigned on-call days provided in advance Schedule Approximately 2-3 assigned on-call days per week Rotating weekend coverage Generally 25-40 hours per week Hours vary based on client schedules and coverage needs Potential overnight coverage as services expand Responsibilities Answer caregiver questions and concerns Manage call-outs, late arrivals, and no-shows Find replacement coverage when needed Respond to urgent staffing or client issues Communicate with caregivers, clients, and families as appropriate Document important changes, incidents, and staffing updates Follow agency escalation procedures Provide direct client care when replacement coverage cannot be found When Providing Client Care Personal care including bathing, dressing, grooming, toileting, and bed baths Mobility, transfers, wheelchair support, and fall-risk supervision Medication reminders, meals, vital signs, and first-aid response within training Dementia-specific cueing, redirection, reassurance, and behavioral support Companionship, light housekeeping, transportation, and appointments when assigned Observation, documentation, care-plan follow-through, infection control, and end-of-life support when appropriate Requirements CNA or nursing background required , including CNA, LPN, RN, or former nurse with hands-on personal-care experience At least 1 year of professional dementia/memory-care experience required Comfortable providing hands-on personal care Strong communication and problem-solving skills Calm, dependable, and able to make appropriate decisions independently Reliable phone access during assigned on-call periods Valid driver's license and reliable transportation Able to travel throughout Weber and Davis Counties Must be able to report to a client if backup coverage is needed Successful completion of required background and driving checks Completion of Forget Me Not Home Care orientation and dementia-care training Travel & Mileage Travel between clients is paid according to company travel and mileage policies. Approved assignments more than 25 miles from your home may qualify for an additional $0.25/mile travel incentive .
