Job Description

Job Summary We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to serve as a Charge Nurse, leading our dedicated nursing team in providing exceptional care to our residents. In this vital role, you will oversee daily clinical operations, coordinate patient care activities, and ensure compliance with healthcare regulations. Your expertise will help create a warm, safe, and supportive environment for individuals with diverse needs, including those requiring memory care, dementia care, and long-term support. This position offers an exciting opportunity to make a meaningful difference in the lives of our residents while demonstrating strong leadership and clinical skills. Duties Supervise and coordinate nursing staff to deliver high-quality patient care across all shifts. Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform care plans. Administer medications, injections, IV infusions, and perform catheterizations following strict protocols and safety standards. Manage complex cases involving acute pain management, infectious disease care, hospice needs, and emergency situations such as airway management or ventilator support. Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner while maintaining strict adherence to HIPAA confidentiality policies. Oversee medical documentation including ICD coding (ICD-9/10), CPT coding, and medical records management for compliance and billing purposes. Collaborate with multidisciplinary teams on diagnostic evaluations, case management, and discharge planning for residents with geriatrics or developmental disabilities. Lead staff training on infection control practices, aseptic techniques, first aid procedures, and emergency response protocols. Ensure compliance with long-term care regulations and quality standards such as NCQA standards while fostering a culture of continuous improvement. Qualifications Valid LVN license in good standing with current CPR/BLS certification. Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly preferred. Demonstrated expertise in long-term care settings including skilled nursing facilities or assisted living communities. Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks; experience working with EMR/EHR management is a plus. Strong knowledge of medical terminology, anatomy, physiology, and medication administration protocols. Experience working with geriatric patients or individuals with dementia or developmental disabilities is highly desirable. Ability to handle complex cases involving acute pain management, tube feeding, ventilator support, or infectious disease control. Excellent communication skills for caregiver communication and client interaction; ability to work effectively within a team environment.
