Office-Based with Field Support as Needed - Santa Ana Office Position Summary The Palliative Care RN Clinical Supervisor provides clinical leadership and oversight for the palliative care program. This position is primarily office-based , focusing on care coordination, clinical oversight, quality assurance, and team support. The RN Clinical Supervisor ensures high-quality patient care, timely service delivery, and compliance with regulatory and organizational standards. While the role is primarily administrative, the supervisor must maintain clinical competency and willingness to provide patient visits in the field when operational needs arise . Key Responsibilities Clinical Leadership & Oversight Provide case management oversight for all patients Provide day-to-day clinical supervision and support to palliative care nurses, nurse practitioners, and interdisciplinary team members. Ensure adherence to palliative care best practices, program protocols, and regulatory requirements. Review clinical documentation for accuracy, quality, and compliance. Provide guidance on symptom management, goals-of-care discussions, and complex patient cases. Run weekly and monthly IDG meeting with contracted health plans and partners Oversee Palliative Care Program QAPI and report to company leadership with support of Program Director and Lead Nurse Pracitioner Staff Support & Development Provide mentorship and clinical support to nursing staff. Assist with onboarding and training of new clinical team members. Participate in performance evaluations and provide feedback to staff. Patient Care Support Maintain readiness to provide direct patient care visits when needed , including admissions, follow-up visits, or urgent clinical support. Support the clinical team during periods of high census, staff shortages, or complex patient needs. Collaboration Work closely with physicians, nurse practitioners, social workers, and other interdisciplinary team members to coordinate patient care. Communicate with referral sources, facilities, and community partners as needed. Work closely with hospice clinical leadership and liaisons to successfully transition patients from one program to another Qualifications Required Active California Registered Nurse (RN) license Minimum 3-5 years of nursing experience Experience in palliative care, hospice, home health, or complex chronic disease management Strong leadership, clinical decision-making, and organizational skills Preferred Prior clinical supervisory or leadership experience Experience working with interdisciplinary care teams Knowledge of palliative care program operations and documentation systems Skills & Competencies Strong clinical judgment and symptom management knowledge Ability to manage multiple priorities in a fast-paced environment Excellent communication and team leadership skills Ability to balance administrative responsibilities with clinical care support
